上肢外骨骼對葡萄疏果作業時降低上臂肌肉活動的功能評估

摘要

研究發現,農業被認為是最危險的產業之一,在所有的職業領域中,工傷、死亡和疾病排名第二。然而農民的職業病中有93%與職業性肌肉骨骼疾病(WMSDs)有關,其中三分之一是腕管綜合症,肩部發生機率是第二高的部位。文獻指出,抬手過肩工作是造成職業性上肢肌肉骨骼疾病(UL-WMSDs)最大的危害因子。當執行抬手過肩工作時,上肢肌肉活動40%最大自主收縮(MVC)不應超過整體工作週期的10%,以避免肩膀疲勞和對肩膀的潛在傷害。 根據研究,在汽車裝配作業廠中執行抬手過肩工作,被動式上肢外骨骼(PULE)可將三角肌的肌肉活動減少34%。鑒於台灣尚未有上肢外骨骼應用於農業工作中的實例,而降低局部肌肉活動被認為是衡量外骨骼性能的重要指標,因此本研究招募6位農民執行葡萄疏果工作30分鐘,並利用肌電訊號(EMG)量化農民使用上肢外骨骼前、後的前三角肌肌肉活動。結果發現,穿戴上肢外骨骼分別可以減少前三角肌靜態、中位數和峰值的肌肉活動83.3%,70.0%和42.9%,且具有顯著差異(P<0.05)。

ABSTRACT

The study found that agriculture is one of the most dangerous industries, with occupational injuries, deaths, and diseases ranking second among all occupational fields. However, 93% of farmworkers' occupational illnesses were related to WMSDs. One-third of the WMSDs cases were caused by carpal tunnel syndrome, and the shoulder was the second most affected component. According to research, the overhand task is a very big risk factor for upper-limb work-related musculoskeletal disorders (UL-WMSDs). When performing the overhand task, the 40% Maximum voluntary contraction (MVC) should not exceed 10% of the work cycle to avoid shoulder fatigue and potential injury to the shoulder. According to research, performing overhead tasks in an automotive factory assembly line, the passive upper-limb exoskeleton (PULE) can reduce the muscle activity of the deltoid by 34%. Since there are no examples of PULE used in agricultural work in Taiwan, reducing local muscle activity is considered an important indicator of exoskeleton performance. Therefore, this study recruited 6 farmers to perform grape thinning for 30 minutes and used electromyography (EMG) to quantify the muscle activity before and after the use of upper limb exoskeletons. The results showed that the PULE reduced the muscular activity of the anterior deltoid, and the static, median, and peak muscle loads were reduced by 83.3%, 70%, and 42.9%, respectively, and with statistical significance (p<0.05).
